Γυμναστική για ένα «Νου Υγιή» - Μέρος 3ο




Από τις πρώτες έρευνες που μελέτησαν τη σχέση άσκησης και διάθεσης είχε παρατηρηθεί η ευφορία που ακολουθεί ένα session έντονης σωματικής δραστηριότητας, φαινόμενο το οποίο ονομάστηκε «jogger's high» ή «runner's high» και το οποίο αποδόθηκε σε διέγερση των οπιοειδών υποδοχέων του εγκεφάλου [37]. Πλέον, όμως, γνωρίζουμε πολλούς άλλους μηχανισμούς με τους οποίους η άσκηση προκαλεί ευθυμία. Οι κύριοι είναι: το σύστημα των ενδορφινών, το σύστημα των κατεχολαμινών και διάφορα νευροδιαβιβαστικά συστήματα.

1) Β – ΕΝΔΟΡΦΙΝΕΣ 
Οι β-ενδορφίνες είναι πεπτίδια, τα οποία συντίθενται κυρίως στην υπόφυση. Απελευθερώνονται στην κυκλοφορία και μεταφέρονται σε άλλες περιοχές του εγκεφάλου μέσω νευρικών ινών, όπου δρουν σε μ υποδοχείς οπιοειδών. Σε καταστάσεις ηρεμίας, τα επίπεδά τους στο αίμα είναι εξαιρετικά χαμηλά. Σε έντονη σωματική άσκηση όμως, αυξάνονται δραματικά ('endorphin rush') και το αποτέλεσμα είναι ένα έντονο αίσθημα ευφορίας. [64], [78]

2) ΚΑΤΕΧΟΛΑΜΙΝΕΣ (ΕΠΙΝΕΦΡΙΝΗ, ΝΟΡΕΠΙΝΕΦΡΙΝΗ)
Η ενεργοποίηση του συμπαθητικού συστήματος κατά την άσκηση αυξάνει σημαντικά τα επίπεδα της νορεπινεφρίνης και της επινεφρίνης στο αίμα. [109] Στον εγκέφαλο, τον κύριο ρόλο τον διαδραματίζει η νορεπινεφρίνη.  Η άσκηση μειώνει τα επίπεδα της νορεπινεφρίνης στο εγκεφαλικό στέλεχος και στο μετωπιαίο φλοιό, παράλληλα μειώνοντας τον αριθμό των φλοιικών β - αδρενεργικών υποδοχέων. Ακόμη, αυξάνει την έκφραση της γαλανίνης, η οποία είναι τροποποιητής της δράσης της νορεπινεφρίνης.
Τα παραπάνω, σε βάθος χρόνου, μειώνουν τις οξείες επιπτώσεις του stress σε διάφορα επίπεδα εγκεφαλικής λειτουργίας, όπως το συναίσθημα και η συμπεριφορά. [73]

3) ΝΕΥΡΟΔΙΑΒΙΒΑΣΤΙΚΑ ΣΥΣΤΗΜΑΤΑ
Αξιοσημείωτη είναι η συμβολή της άσκησης στην αυξημένη έκφραση γονιδίων (upregulation of genes) με αποτέλεσμα ενίσχυση της γλουταματεργικής νευρομεταβίβασης. [37] Το τρέξιμο έχει φανεί πως μπορεί να αυξήσει τους υπότυπους του NMDA υποδοχέα (βασικός συντελεστής της γλουταματεργικής διαβίβασης), και των υποδοχέων NR2A και NAR2B, σε ιππόκαμπους πειραματόζωων. Επίσης, πάλι σε πειραματόζωα και στο πλαίσιο χρόνιας άσκησης, ενισχύει τη μεταβίβαση νευρικών σημάτων μέσω του συστήματος των μονοαμινών, αυξάνει ελαφρώς τα βασικά επίπεδα σεροτονίνης – η σεροτονίνη προκαλεί ευφορία – στο ραχιαίο πυρήνα της ραφής, και αυξάνει το ισοζύγιο σεροτονίνης στον εγκεφαλικό φλοιό. [37]
Όλοι οι παραπάνω μηχανισμοί φαίνεται να έχουν πολύ θετικό αντίκτυπο στη βελτίωση της διάθεσης και στη μείωση καταθλιπτικών διαταραχών, ενώ η ενίσχυση της γλουταματεργικής διαβίβασης έχει θετικές επιπτώσεις και στη μνήμη. [37]


ΕΡΕΥΝΕΣ ΣΕ ΑΝΡΘΩΠΟΥΣ 
Πολλές έρευνες μελέτησαν απευθείας την αντικαταθλιπτική δράση της σωματικής άσκησης σε ανθρώπους, ιδίως μακροχρονίως. Από προοπτικές έρευνες έχει δειχθεί ότι άτομα που δεν ασκούνταν στην παιδική ηλικία έχουν μεγαλύτερη πιθανότητα να εμφανίσουν καταθλιπτικά συμπτώματα μετέπειτα σε σχέση με άτομα πιο δραστήρια. [41] Ο Penninx και οι συνεργάτες του ανέφεραν σημαντική μείωση καταθλιπτικών συμπτωμάτων σε 439 ηλικιωμένους, οι οποίοι συμμετείχαν σε 18μηνο πρόγραμμα περπατήματος [76].
 Η Anne και οι συνεργάτες της, από την πλευρά τους, χώρισαν σε 2 ομάδες ένα δείγμα 86 ατόμων, 53 – 78 ετών. Η μια ομάδα ακολούθησε ένα πρόγραμμα σωματικής άσκησης, επιβλεπόμενο, για 10 εβδομάδες και η άλλη παρακολουθούσε εκπαιδευτικά σεμινάρια σχετικά με την ψυχική υγεία. Στο τέλος των 10 εβδομάδων, το 55% των ατόμων που ήταν στην ομάδα σωματικής άσκησης βίωσε περισσότερο από 30% μείωση στην κατάθλιψη, ενώ στην άλλη ομάδα μόνο το 33% ανέφερε βελτίωση. Η σωματική άσκηση, δηλαδή, ίσως υπερτερεί της απλής κοινωνικής παρέμβασης στην αντιμετώπιση της κατάθλιψης. [5]
Με τη σειρά τους, ο Babyak και οι συνεργάτες του έδειξαν ότι ασθενείς με καταθλιπτική διαταραχή που γυμνάζονται έχουν μικρότερες πιθανότητες να επαναλάβουν καταθλιπτικό επεισόδιο μετά από 10 μήνες, κυρίως αν συνεχίζουν τη γυμναστική κατά την περίοδο follow-up [9], ενώ η άσκηση αποδεδειγμένα μειώνει καταθλιπτικά συμπτώματα που απορρέουν από την ύπαρξη χρόνιων ασθενειών [113]. Τέλος, τα αποτελέσματα της  Almada County Study έδειξαν ότι η φυσική άσκηση συσχετίζεται με μειωμένο επιπολασμό και επίπτωση της κατάθλιψης σε διάστημα 5 ετών σε ένα δείγμα 1947 ενηλίκων, 50 έως 94 ετών. [107]


ΑΣΚΗΣΗ VS ΑΝΤΙΚΑΤΑΘΛΙΠΤΙΚΑ ΦΑΡΜΑΚΑ 
Γνωρίζουμε ότι  αντικαταθλιπτικά φάρμακα τύπου SSRIs όπως η φλουοξετίνη αυξάνουν τα επίπεδα σεροτονίνης στον εγκέφαλο και ενισχύουν τη νευρογένεση. Το ίδιο ακριβώς κάνει και η άσκηση, με τη διαφορά όμως ότι έχει θετική επίδραση στη νευρογένεση σε σύντομο χρονικό διάστημα ενώ η φλουοξετίνη απαιτεί χρόνια χορήγηση και υπάρχει μεγάλη ανομοιογένεια στο νευρογενετικό αποτέλεσμα της φλουοξετίνης ανά άτομο. Επίσης, τα ποσοστά υποτροπής των ασθενών με κατάθλιψη είναι πολύ μικρότερα σε βάθος χρόνου σε όσους ασκούνται απ' ότι σε όσους παίρνουν αντικαταθλιπτικά φάρμακα μόνον. [37] Σε απευθείας σύγκριση, η άσκηση έχει αποδειχτεί εν γένει καλύτερη ως προς την αντικαταθλιπτική της δράση έναντι φαρμάκων όπως το Prozac (η εμπορική ονομασία της φλουοξετίνης) [31] ενώ μια τυχαιοποιημένη κλινική έρευνα από τον Blumenthal και τους συνεργάτες του έδειξε ότι η χορήγηση σερτραλίνης (50mg έως 200mg), η σωματική άσκηση (με τη μορφή περπατήματος-χαλαρού τρεξίματος, 3 φορές την εβδομάδα, για 16 εβδομάδες) και ο συνδυασμός των δύο, είχαν όλα αντικαταθλιπτική επίδραση στους συμμετέχοντες. Το αξιοσημείωτο είναι ότι δεν υπήρχε στατιστικά σημαντική διαφορά ανάμεσα στις 3 ομάδες των ατόμων, δηλαδή και πάλι η άσκηση δεν υστερούσε απέναντι στο αντικαταθλιπτικό φάρμακο. [11]

Γενικά η γυμναστική έχει έντονη αντιστρεσογόνο δράση [23], ιδίως όταν γίνεται χρονίως, και οι μηχανισμοί είναι διάφοροι [7], [67], [75], [85]:
1) Ποιοτικότερος ύπνος και καλύτερη ξεκούραση (ίσως φαίνεται περίεργο αλλά η αύξηση της σωματικής δραστηριότητας μπορεί να μειώσει τα συμπτώματα κόπωσης [62], [81])
2) Αύξηση της libido
3) Καλύτερη αντοχή και μυϊκή δύναμη
4) Αύξηση ετοιμότητας-εγρήγορσης
5) Μείωση βάρους και βελτίωση αντίληψης για την εικόνα του σώματος

Παρ’ όλα αυτά, σημειώνεται ότι σε μια συστηματική ανασκόπηση της βάσης δεδομένων Cochrane το συμπέρασμα ήταν ότι η άσκηση όντως φαίνεται να μειώνει τα καταθλιπτικά συμπτώματα σε διαγνωσμένα με κατάθλιψη άτομα, αλλά όταν ληφθούν υπόψιν μόνον οι μεθοδολογικά συμπαγείς μελέτες, τα αποτελέσματα μετριάζονται πολύ και συχνά δεν είναι στατιστικά σημαντικά. Συνεπώς, απαιτείται περισσότερη έρευνα πάνω στην επίδραση της άσκησης σε ανθρώπους με δυσθυμία. [63]


ΠΡΟΤΑΣΕΙΣ ΓΙΑ ΣΩΜΑΤΙΚΗ ΑΣΚΗΣΗ ΣΕ ΗΛΙΚΙΩΜΕΝΟΥΣ
Το Center for Disease Control and Prevention (CDC) προτείνει αυξημένο επίπεδο σωματικής δραστηριότητας ενσωματώνοντας οποιαδήποτε δραστηριότητα έστω και μέτριας έντασης μέσα στη μέρα. Για ηλικιωμένους, προτείνεται η συνολική σωματική δραστηριότητα να γίνεται περιστασιακά με προτεινόμενες μορφές ασκήσεων τη μέσης έντασης αεροβική προπόνηση (πιο δημοφιλές είναι το περπάτημα), τη γυμναστική μυϊκής ενδυνάμωσης και τις ασκήσεις ισορροπίας και ευλυγισίας. Επιπλέον, προτείνεται σαφέστατα η οργάνωση ενός προγράμματος σωματικής άσκησης στα πλαίσια πρόληψης και θεραπείας της νόσου Parkinson (σε συνδυασμό με χορήγηση levodopa). [1], [72]
Αξίζει να σημειωθεί εδώ πως, σύμφωνα με το US Department of Health and Human Services, οι ηλικιωμένοι άνθρωποι, γενικά ενδιαφέρονται πάρα πολύ να κάνουν αλλαγές στην καθημερινότητα και τη συμπεριφορά τους με σκοπό να διατηρήσουν την υγεία και την αυτονομία τους και συνεχώς αναζητούν πληροφορίες με αυτόν το σκοπό [114].  Έχει παρατηρηθεί, μάλιστα, πως έχουν μεγαλύτερη συμμόρφωση προς παρεμβάσεις που τους επιτρέπουν κάνουν μορφές σωματικής άσκησης που οι ίδιοι προτιμούν, σε περιβάλλοντα ευχάριστα προς αυτούς και μη ανταγωνιστικά [44] ενώ τη συμμόρφωση επηρεάζουν και όπου νιώθουν ικανοί και ασφαλείς και όχι κάτω από συνθήκες ανταγωνισμού. Άλλοι παράγοντες που καθορίζουν τη συμμόρφωση προς ένα πρόγραμμα άσκησης είναι: βιοατρικό status, παλαιότερη ενασχόληση με άθληση και μορφωτικό επίπεδο. [12] Τέλος, παρεμβάσεις που μπορούν να λάβουν χώρα μέσα στο ίδιο το σπίτι των ηλικιωμένων ή παρεμβάσεις που περιλαμβάνουν έλεγχο με τηλεφωνικές επαφές και ελέγχους συμπεριφοράς έχουν δείξει ενθαρρυντικά αποτελέσματα. [45, [116]


ΣΥΜΠΕΡΑΣΜΑ
Η ανασκόπηση της σύγχρονης βιβλιογραφίας γύρω από τον τρόπο με τον οποίο η σωματική άσκηση επηρεάζει το κεντρικό νευρικό σύστημα οδηγεί σε ένα ξεκάθαρο συμπέρασμα: η σωματική άσκηση αποτελεί έναν αποδεδειγμένα αποτελεσματικό τρόπο για τη βελτίωση και την προστασία της διανοητικής υγείας, σε επίπεδο γνωσιακό, θυμικό, και σε επίπεδο γενικότερης ευρωστίας καθόλη τη διάρκεια της ζωής μας. [10], [97]. Μάλιστα, είναι μια παρέμβαση η οποία μπορεί να προσαρμοστεί στις ανάγκες του καθενός, χωρίς ιδιαίτερο κόστος και χωρίς ιδιαίτερες παρενέργειες ή κινδύνους. Πρέπει, βέβαια, να υπογραμμιστεί η έλλειψη μεγάλων τυχαιοποιημένων κλινικών ερευνών οι οποίες θα μελετούσαν πιο ενδελεχώς τις επιδράσεις της άσκησης και επισημαίνεται πως θα πρέπει στο μέλλον να πραγματοποιηθούν τέτοιες έρευνες ώστε να προκύψουν πιο συμπαγείς αποδείξεις για την αποτελεσματικότητά της.

Πάντως, μπορούμε με ασφάλεια να συμπεράνουμε ότι η αρχαία ρήση «Νους υγιής εν σώματι υγιή» εκπληρώνεται και στα 2 της σκέλη από τη γυμναστική, η οποία κάνει αυτό ακριβώς: διατηρεί το νου μας υγιή μέσα σε ένα υγιές, δυνατό και καλλίγραμμο σώμα. Τελικά, φαίνεται πως ισχύει η ρήση του Ιπποκράτη για τη φυσική άσκηση: "το περπάτημα είναι το καλύτερο φάρμακο του ανθρώπου". [36]


Προσωπικό σχόλιο
Παρόλο τον τόσο μεγάλο όγκο βιβλιογραφίας υπέρ της ευεργετικής επίδρασης της γυμναστικής σε διάφορους τομείς, δεν έχει αλλάξει ιδιαίτερα η νοοτροπία απέναντί της από τον κόσμο. Συχνά αντιμετωπίζεται ως μια απλή «καλή συνήθεια», ως ένα απλό «επικουρικό» εργαλείο στη (φαρμακευτική) θεραπεία ασθενειών, ενώ σε πιο ακραίες καταστάσεις ως πράξη ματαιοδοξίας και ναρκισσισμού (όχι ότι δεν μπορεί να είναι, αλλά αυτό σε καμία περίπτωση δεν επισκιάζει τα οφέλη της, εκτός ακραίων καταστάσεων). Μεγάλη η ευθύνη της ιατρικής κοινότητας σε αυτή τη λανθασμένη αντίληψη περί της άσκησης. Πολλοί σύγχρονοι γιατροί είτε λόγω ελλιπούς ενημέρωσης είτε επειδή θέλουν να προσφέρουν την εύκολη λύση στον ασθενή είτε λόγω και των 2, αρκούνται στο να «προτείνουν» την άσκηση δίνοντας έμφαση μόνο στα φάρμακα, ενώ στην πραγματικότητα και η ίδια η άσκηση, σε πολλές περιπτώσεις, πρέπει να συνταγογραφείται. Άλλωστε, τα ίδια τα δεδομένα δείχνουν ότι  όχι μόνο δεν υστερεί, αλλά υπερτερεί ενίοτε έναντι των φαρμάκων και σε αποτελεσματικότητα και σε κόστος και σε επίπεδο παρενεργειών. Πρέπει κάποια στιγμή αυτός ο τρόπος σκέψης να αλλάξει και να γίνει επιτέλους συνειδητό ότι η άσκηση είναι φάρμακο, φάρμακο πρόληψης και θεραπείας, το οποίο είναι ευκταίο να «λαμβάνεται» από όλους.)
